Methylphenidate for ADHD: Uses, Side Effects & Safety

Methylphenidate is one of the most widely used and studied stimulant medications for ADHD. Brand names include Ritalin and Concerta. It is a prescription-only, Schedule II controlled substance and is not sold through our online store; it may be prescribed by a licensed provider after an evaluation.

How Methylphenidate Works

Methylphenidate is a central nervous system stimulant. It increases the activity of dopamine and norepinephrine in the brain by blocking their reuptake, which helps improve attention, focus and impulse control.

Forms and How Long They Last

  • Immediate-release tablets (such as Ritalin) usually last about 3–4 hours and may be taken two or three times a day.
  • Extended-release forms (such as Concerta and Ritalin LA) last roughly 8–12 hours and are usually taken once in the morning.
  • Other forms include chewable tablets, oral solutions and a skin patch.

Dexmethylphenidate (Focalin) is a closely related medication.

Common Side Effects

  • Decreased appetite and weight loss
  • Trouble sleeping
  • Headache or stomach ache
  • Nervousness, irritability or anxiety
  • Increased heart rate and blood pressure

Side effects are often mild and may improve as your body adjusts or with a dose or timing change made by your provider.

Important Safety Information

  • Methylphenidate carries a boxed warning for abuse, misuse and addiction. Take it only as prescribed and never share it.
  • Tell your provider about heart problems, high blood pressure, mental health conditions, glaucoma, tics or any history of substance use.
  • Do not take it within 14 days of an MAO inhibitor.
  • Seek medical help right away for chest pain, fainting, shortness of breath, new or worsening mood changes, or an erection lasting more than 4 hours.

This is not a complete list. Read the medication guide that comes with your prescription and ask your pharmacist or provider about anything you don’t understand.

Controlled substance rules

As a Schedule II medication, methylphenidate cannot be refilled. Each fill requires a valid prescription, and federal and state telehealth rules may require additional steps.

Is Methylphenidate Right for You?

Only a licensed provider can decide. If stimulants aren’t a good fit, a non-stimulant such as atomoxetine may be considered. Start with an online ADHD assessment.
