How the Online ADHD Assessment Works

  1. Request your assessment. Tell us a little about yourself and our care team will contact you to get started.
  2. Complete the questionnaire. It takes about 10 minutes and uses standardized adult ADHD screening questions about attention, organization, restlessness and impulsivity.
  3. Meet a licensed provider. In a secure video visit, your provider reviews your answers, childhood and current symptoms, medical history and other conditions that can mimic ADHD.
  4. Get your results and plan. If ADHD is diagnosed, your provider recommends a personalized treatment plan. If not, they’ll help you understand what else may be going on.

  • Sleep, mood, anxiety and substance use, which can affect focus
  • Medical history, current medications and allergies

How to Prepare

  • Write down your main symptoms and examples of how they affect you.
  • Gather any past school reports, evaluations or diagnoses if you have them.
  • List your current medications, supplements and health conditions.
  • Have a valid photo ID and a private place for your video visit.

What a Screening Can and Can’t Do

An online questionnaire alone cannot diagnose ADHD. It helps you and your provider understand your symptoms. A diagnosis is made only after a clinical evaluation by a licensed provider.

No guaranteed prescriptions

Medication is prescribed only when it is clinically appropriate and permitted by law. Stimulants are controlled substances, so additional steps may be required.
